This is a market engagement exercise where the Commissioner seeks your views for this requirement and asks a number of set questions.

This is a process designed to help the Commissioner form a view of the best way to commission the service and is not the beginning of a Tender exercise. A further Tender advertisement will be issued at the appropriate time where required. Your feedback at this point will not have a bearing on any future Tender submissions you may wish to offer at a later date. You will not be disadvantaged if you choose not to respond to this RFI but it will be helpful to understand your views at this early stage, so you are encouraged to respond as fully as you can.

The aim of the service is to provide a telephone-based behavioral coaching service to support patients with long term conditions improve their ability to self-manage and help to reduce avoidable accident and emergency attendances and hospital admissions.

This will be achieved by offering advanced coaching and motivational interviewing techniques over the telephone. Patients can become more confident at managing those issues that have become a block to attaining and sustaining lifestyle changes that in turn improve health outcomes.

Aimed at people who:

o Are adults (aged 18+) who have:

o been newly diagnosed with a long-term condition

o or who have not engaged with self-managing their long term condition/making healthy behavioral changes
