Communities of Practice can be a highly effective enablers of profound change - offering a foundation of network from which people can build learning capabilities, enhance personal results, create new business practices and deliver enhanced business results - for patients, staff and communities.

In 2016/17 communities of practice were held across the new models of care, multiple cohorts. These new models of care communities of practice allowed sites to access peer support, alternative views and perspectives and access tools. A positive impact from the communities of practice was recognised as a result of participants enhancing their knowledge of existing and emerging good practice, time and facilitates in which to share experiences and enabled the building of personal resilience in the face of uncertainty.

This proposal is for the continued development of the communities of practice established for the new care model cohorts - Acute Care Collaboration (ACCs), Multi-specialty Community Provider (MCP) and Primary and Acute Care Systems (PACS) and their evolution within the developing health care systems. This opportunity is to design the next programme of communities of practice and is offered in 3 lots.

These communities of practice will seek to accelerate leadership learning and capability building, enhance leadership practice and enable the rapid delivery and spread of good practice within the new care models and their evolution within developing healthcare systems.

These communities of practice will achieve the following, underpinned by the specialisms appropriate to each lot:

a. Identify and capture the learning of the new care models.

b. Create engaging narratives that accurately describe good practice and provides supporting guidance.

c. Through networking, achieve rapid spread of the learning to other vanguards and the translation of this learning to the developing wider healthcare systems and STP geographies.

d. Build the capability of the communities of practice to continue to forge bonds for their collective learning beyond 2016/17.
